How the Two-Day D&E Procedure Works

Most patients complete second trimester abortion care in two appointments across two days. This allows the cervix to open gradually overnight, supporting a safe and gentle process.

Day 1: Pre-Procedure Appointment

On the first day, you will meet your care team and review your medical history. You’ll have time to ask questions, meet with a nurse and physician, and receive an ultrasound to confirm gestational age.

The physician will administer medications to begin cervical preparation. If needed, we may perform a pelvic exam to place Dilapan, a sterile synthetic dilator (similar to laminaria) that helps the cervix open slowly overnight.

  • Light to moderate cramping may occur overnight
  • Mild bleeding can be normal
  • We provide pain medications to keep you comfortable

Day 2: Procedure & Recovery

When you return, you will receive IV medications to help you feel comfortable and relaxed and to control pain. Once you are comfortable, the physician will complete the procedure—typically 15 minutes or less.

We use ultrasound guidance during procedures to support safety. Afterward, you will recover in the privacy of your own room. Your partner, family, or friends may be able to be with you during recovery.

Typically, anesthesia medications wear off within two hours and patients can return home or to their hotel.

Can This Procedure Take Three Days?

In most cases, abortion care from 17–25 weeks is completed in two days. In select cases—usually after 24 weeks and depending on medical history—a three-day plan may be recommended to support safe cervical preparation. Your physician will explain your individualized plan during your first appointment.
